The Hemlock Pain Center has successfully treated all types of chronic pain in all parts of the human body. Our doctors are board certified anesthesiologists who have dedicated themselves to the management of chronic pain and regularly receive additional subspecialty training in pain management.
The Hemlock Pain Center is a referral-based facility. This means that a referral is required from a physician such as your Primary Care Provider or a Specialist – one who knows your medical history and with whom you have an established visit history. Emergency Room physicians would not be applicable in this case. We do not accept self-referrals.
Once the referral is received, our physicians review the information included in the referral to determine whether or not our facility can be of assistance. If our services can meet your specific needs, you will be contacted to schedule an appointment. Please ensure that your referring provider has your most current information including address, insurance, and most recent test results.
We accept most major insurance plans, and will verify your insurance coverage when we contact you to schedule your first appointment.
Under normal circumstances, we can take patients within a week of the referral – not months like some specialty clinics. In severe/emergent cases, we can see new patients in as little as 24 hours or same day with an emergency call from the referring physician.
When a referral is received and you are determined to be a candidate for our services, the Hemlock Pain Center will contact you to schedule an appointment for your first visit. At that time, your address will be verified in order for us to mail a new patient packet, or you may stop by our office to pick up the packet.
Please ensure you complete this packet in its entirety prior to your appointment time. Failure to do so may cause a significant delay in being seen by your appointment provider.
We perform random urine screens as required by law. Please be prepared to provide an on-site urine sample at every visit.
